Responsibilities:
In this role of a Field Case Manager you are responsible for completing assessments with member/caregiver/provider to obtain information regarding client status, support system, and need for services for care plan development
Monitor delivery of services and follow-up with members/caregivers/providers through member face to face
Coordinate services between the PCP specialist and other medical and non-medical providers as necessary to meet the needs of clients.
Assist in coordinating the development of informal or voluntary services to integrate into the member care plan

Hours for this Position:
Day time hours - Monday-Friday - 8:00 am - 5:00 pm

What We Look For:
o Thorough knowledge of specialized field such as geriatric, adult social work or long term care
o Manages 60-80 patients per caseload
o Training will be in 4 weeks in the Tampa, FL office

The check will bounce and the victim is left responsible.
